Is there any reason why it wouldn't work to give everyone say an extra $5K per year from Social Security, while at the same time adding a $5K deductible to Medicare? If you need a higher level of health care, you're in exactly the same position as before. If not, you're incentivized to avoid unneeded care so you can keep the cash. Everybody wins.
